All Candidates for WHS Sports:

Carroll County Public Schools Athletics would like to inform parents and prospective student-athletes of a procedural change regarding paperwork for participation in athletics.  Beginning in the Fall of 2015, in order to participate in athletics at Westminster High School, all candidates must complete the Carroll County Athletic Forms Packet and turn it in to the head coach or the Athletic Director, Mr. Terry Molloy.   All of the required paperwork needed for athletic participation can be found at the following locations:

Student-athletes/parents must print out and complete all documents in the link entitled Required Paperwork.  It is also very important for student-athletes/parents to read all documents in the link entitled Informational Packet. The Required Paperwork packet must be completed, signed by a physician, parent and student and returned to the head coach prior to or on the first day of try-outs.  If your physical form from the previous year is still valid (less than a year old) indicate that on the 3rd page of the packet.  If you do not have the ability to print off the required paperwork, packets are available in the main office of the school.  You may not turn these packets in to the main office. 

Concussion Testing: All candidates must also have completed concussion baseline testing prior to the first day of tryouts. Testing is good for two years from the date of the test.
